# The Medicare Safety Net Resets on January 1
If you're considering further treatment, there's a feature of the Medicare system worth factoring into your timing. This article explains how it works so you can make an informed decision.
## How the Extended Medicare Safety Net works
Medicare contributes to most stages of an IVF cycle, but not the full cost. The difference between the Medicare benefit and the cost of your treatment is your out-of-pocket expense.
The Extended Medicare Safety Net (EMSN) keeps a running total of those out-of-pocket costs for out-of-hospital services across a calendar year. **Once your total passes a threshold, Medicare pays 80% of full capped amount of your further out-of-pocket costs for the rest of that year.**
There are two thresholds — a lower one for concession card holders and families receiving Family Tax Benefit Part A, and a higher one for everyone else. Both are indexed on 1 January, so the exact amounts change each year. Current figures are published by Services Australia.
## The tally resets on 1 January
Your Safety Net total returns to zero at the start of each calendar year. It doesn't carry over.
The practical effect is that treatment costs are not evenly distributed across the year. **If you have already had treatment in 2026, you may have passed the threshold, in which case further treatment this year would attract the higher benefit. From January, that count starts again.**
This is worth knowing if you are weighing up timing. It is one factor among several, and for many people it will not be the deciding one.
## Other things that affect the calculation
**Family registration.** Couples' costs are only combined for Safety Net purposes if you are registered as a family with Services Australia. This is a separate step from being on Medicare together and is often missed.
**Caps on ART items.** Assisted reproductive technology items have a maximum EMSN benefit per item, so the 80% is not unlimited. The saving is real but may be smaller than the headline figure suggests.
**In-hospital services.** The Safety Net applies to out-of-hospital services. Procedures performed in a day hospital, such as egg retrieval, are treated separately and may be covered by private health insurance.
**Medications.** IVF medications are subsidised through the PBS, which has its own separate safety net and threshold.
**Lead time.** Consultations, any additional testing and cycle planning typically take several weeks, so treatment can't usually be arranged at short notice.
## Checking your own position
You can view your Safety Net status through your Medicare online account via myGov, or the Express Plus Medicare app.
